IMPORTANT INFORMATION

Please carefully read through the points below before filling out and submitting this form.

The full delivery period for Summer 2025 will be

Wednesday, 23rd July 2025 - Friday, 29th August 2025

Please complete this form to let us know if you would like to place an order to purchase Sanitary Boxes for delivery during the first 3 weeks of Summer.

A Sanitary Box is priced at £64 per box and contains almost 300 period products, which includes tampons and sanitary pads. Delivery is included in the price above.

Please fill out this order form in its entirety, detailing the menu items you would like to purchase, and information required for delivery including primary and secondary contact details, delivery dates, hours of operation, preferred delivery windows, and any delivery access/ special instructions.

Please clearly indicate the number of Sanitary Boxes you would like to purchase.

Once all order forms have been received by the deadline and processed, we will be in touch to confirm receipt of your order and send a purchase agreement requiring signature to finalise your order. Please be sure to be checking your spam folders to avoid missing any communications from us in this regard.

Please be advised that once the purchase agreement has been signed, we will be unable to make any changes to the order list and you will be financially responsible for the entire order.

The deadline for completing and submitting an order for the 1st half of Summer 2025 is Monday 16th June by 5pm.

MAILCHIMP NOTICE OF CONSENT

At the Mayor's Fund for London, we use Mailchimp as our email marketing and SMS platform and we will use it to communicate with you about your orders and deliveries by email and text message.

Once your order has been finalised, we will send order summary details by email and delivery reminders by SMS to the primary and secondary contacts you indicate on this form up to two weeks before the start of the delivery period and during week of scheduled delivery.

PLEASE NOTE:

The general delivery window is 09:00 to 17:00.

We strongly encourage you to have staff onsite during these hours on your requested delivery day(s). If you will not have staff available and onsite throughout on your delivery day, you will need to let us know prior to delivery day of a suitable and nearby staffed address location for the delivery team to leave your delivery at in order to avoid missing your delivery.

Please be advised that any missed deliveries, last minute delivery cancellations or redelivery requests may result in missed delivery/ redelivery charges being incurred and additionally billed for.

We also ask that you ensure the availability of enough staff or helping hands to receive your delivery, especially if your site location is not easily accessed.

We may follow up to request alternate delivery days and times if our delivery team cannot meet your preferred choices/ selections.
